Office Policy. Must be signed by all new patients. This document explains our policies and procedures regarding payment from clients with and without insurance coverage, telephone calls, and cancellations.
New Patient Questionnaire. For new patients and patients with new problems/concerns. Gives us valuable information needed to help you, including personal information and medical history, your habits, current signs and symptoms, and family medical history.
Patient Privacy
FirstHealth of Andover PC is compliant with the Health Insurance Portability & Accountability Act of 1996 (HIPPA). Patients will all receive the notice of privacy practices, which contains a complete description of the uses and disclosures of any health information.
